04 Depression, bipolar disorder, and related disorders, meeting A and B, or A and C: Medical records documenting symptoms in paragraph 1 or 2: Depression with five or more of the following symptoms: Depression; Loss of interest in almost all activities; Weight changes causing appetite disturbances; Sleep disturbances; observable psychomotor agitation or retardation; Reduced energy; Feelings of guilt or worthlessness; trouble concentrating or thinking; or Thoughts of death or suicide.


Suffering from bipolar disorder with three or more of the following: stressed speech; The flight of thoughts; Inflated self-esteem; Decreased need for sleep; Distraction; Engaging in afghanistan phone number list that have an unrecognized high likelihood of causing painful consequences; or Increased goal-directed activity or psychomotor agitation. Criteria for Parts B and C of the Depression Inventory and Extreme limitation in one or two of the following areas of mental functioning: Understand, remember, or apply information.

Adapt or manage yourself. or Your mental disorder in this list category is "severe and persistent"; that is, you have medical records showing the disorder for at least 2 years, and there is evidence that: Medical treatment, mental health treatment, psychosocial support, or an ongoing, highly structured environment that reduces the signs and symptoms of your mental condition; and Marginal adjustment, that is, your ability to adapt to changes in your environment or demands that are not part of your daily routine is minimal.
